Medicine Reminder Chart

Make a clear medicine chart for yourself or for a parent or grandparent. List each medicine with the dose to take in the morning, afternoon, evening and night, and whether it is before or after food. The PDF shows the whole day at a glance and a tick-off grid, so nobody forgets or doubles a dose. A big-print option is there for elders.

How do I fill the doses?

For each medicine type how much to take in the morning, afternoon, evening and night - for example 1, 2, 1/2 or 5. Choose what the dose is in (tablets, ml and so on) and the tool writes "1 tablet", "2 tablets" or "5 ml". A time left empty means the medicine is not taken then. A prescription written as 1-0-1 means 1 in the morning, none in the afternoon and 1 at night.

What do I get in the PDF?

A "day at a glance" with a coloured box for the morning, afternoon, evening and night listing what to take at each time, a table of all medicines with the food instruction and the number of days, and a tick-off sheet with one box for every dose on every day. Boxes after a medicine's last day are greyed out.

Does the tool tell me the right dose?

No. It only arranges what you type, which should come from the doctor's prescription. It does not check doses, interactions or allergies. Never change or stop a medicine without asking your doctor or pharmacist.

Can I print it in big letters for my parents?

Yes. Switch on "Big print for elders" and the day-at-a-glance and the table are printed in a larger size. Print the first page and stick it where the medicines are kept.

How does the tick-off sheet work?

Each row is one medicine at one time of the day, and each column is one day, starting from the date you choose. Print it and tick the box after every dose. Choose 7 days, 14 days or a full month (31 days). For a longer course print another sheet with a later start date.

Is my information stored?

No. The details are used only to draw the PDF and are not saved on our server. A copy of the form stays in your own browser so you do not lose it if you refresh the page.
