BP & Sugar Log

Keep one tidy record of your blood pressure, pulse and blood sugar. Enter the name, date of birth and gender once, then add a row for every check. The PDF shows each reading with its status (normal, elevated, high, low), the averages, and graphs that show how you are doing over time - handy to carry to the doctor.

What do I have to fill in?

The name, date of birth and gender, and at least one reading. Every reading needs a date plus a blood pressure, a pulse or a sugar value. For blood pressure enter both the upper and the lower number - if one is missing the pressure is left out. Rows with no date or no reading are left out of the PDF, and the download button tells you what is missing.

What are the normal ranges used?

Blood pressure follows the commonly used adult ranges: normal is below 120/80, elevated 120-129 (with a normal lower number), high stage 1 is 130-139 or 80-89, high stage 2 is 140/90 or more, and very high is 180/120 or more. Below 90/60 is shown as low. Sugar: fasting under 100 mg/dL is normal, 100-125 is the prediabetes range and 126 or more the diabetes range; after a meal (or random) under 140 is normal, 140-199 prediabetes range and 200 or more diabetes range. Below 70 mg/dL is low.

What does "after meal (PP)" mean?

PP means post-prandial - a sugar test taken about two hours after you start eating. Choose Fasting if you tested after at least 8 hours without food, and Random for any other time.

Why are there no status labels for a child?

The ranges above are for adults aged 18 and over. Children have different ranges by age and height, so for them the PDF shows the readings only. Ask the paediatrician what is right for the child.

Can I use mmol/L for sugar?

Yes. Choose mmol/L in the Units section and type your readings in that unit. The ranges are converted for you (18 mg/dL is about 1 mmol/L).

Is this medical advice?

No. It is a tidy record for personal tracking, and one reading never makes a diagnosis. Readings change with rest, food, medicines and stress. Show the sheet to your doctor and follow their advice. If a reading is very high or very low and you feel unwell, get medical help at once.

Is my information stored?

No. The details are used only to draw the PDF and are not saved on our server. A copy of the form stays in your own browser so you do not lose it if you refresh the page.
